Background
Routine sources of information on the maternal and child health workforce in China are without clear definition and categorisation. The aim of the study was to systematically review all the evidence on China's maternal and child health workforce profile (ie, level of education, training, qualification, and professional title), and determine the density of the maternal and child health workforce.Methods
We did a systematic review by searching six English (Embase, MEDLINE, CENTRAL, EconLit, Global Health, and Web of Science) and two Chinese (Wanfang and China National Knowledge Infrastructure) databases, from 1949 onwards, using a combination of the search terms “human resources for health”, “maternal and child health services”, and “China” with both thesaurus and free text words. We included studies either describing the profile of the maternal and child health workforce or providing data allowing us to calculate the density of the maternal and child health workforce.Findings
We included 58 studies: 43 reporting profiles of the maternal and child health workforce, and 19 reporting density of the maternal and child health workforce, four of which covered both. 51 (88%) of the 58 studies were done after 1990. The maternal and child health workforce in China covers an array of professions, including obstetricians, gynaecologists, neonatologists, paediatricians, nurses, midwives, general physicians, specialised public health workers, vaccinators, barefoot doctors (ie, farmers who go through short-term medical training), and traditional birth attendants. Definitions of who qualifies as a maternal and child health provider are not clear (eg, the term midwife was used in six studies, and covered a range of training, including clinical medicine, maternal and child health care, nursing, and midwifery). Two studies reported that 7% (24 of 321) and 48% (650 of 1364), respectively, of the maternal and child health workforce at county-level facilities or below held no certificate for maternal and child health care. Only one study reported the density of the maternal and child health workforce at a national level, which was 0·6 health professionals per 1000 population in 2011. The density of the maternal health workforce was between 1·6 and 6·5 times higher than the child health workforce in the same population. The ratio of obstetric nurses to obstetricians ranged from 1·3:1 to 2·0:1, which was higher than the overall nurse-to-doctor ratio at a national level of 1·1:1 in 2017. The ratio of paediatric nurses to paediatricians ranged from 1·1:1 to 1·7:1, which was higher than the national ratio of 1·1:1.Interpretation
The density of the maternal and child health workforce in China is lower than the minimum desired level of 2·3 health professionals (physicians, nurses, and midwives) per 1000 population, as recommended in the World Health Report 2006. The maternal and child health workforce in China is characterised by varied personnel with diverse training backgrounds, a larger maternal health workforce than child health workforce, and more nurses than doctors. A strength of the study is the conceptual understanding of the maternal and child health workforce over the entire period of contemporary China. A limitation of the study is that various data sources prevented us from synthesising the available evidence together.Funding

A public health research system is the bedrock of health systems to improve population health, system responsiveness, and equity. An international concern, referred to as the 10/90 gap, is that less than 10% of global funds are devoted to diseases or conditions that account for 90% of the global disease burden, particularly in developing countries. Palestinian health research is progressing, but it is not sufficiently investigated, with a remarkable knowledge gap on its conceptualisation, stewardship, stakeholders, and capacity and resources. The aim of this study was to understand the Palestinian public health research system by investigating challenges related to the system components that need to be strengthened.Methods
The study was done in the Gaza Strip and West Bank in the occupied Palestinian territory between January and July, 2016. We targeted relevant government institutions, academic schools, and large local and international health agencies. Data were collected through 52 in-depth interviews and six focus group discussions with policy makers, academics, and experts. Participants and institutions were selected purposively on the basis of stated criteria and peer review. Data were translated, transcribed, checked, and imported into MAXQDA 12 for thematic and content analysis. Approvals were obtained from The Research Commission of Swiss TPH, “Ethikkommission Nordwest- und Zentralschweiz” (EKNZ) in Switzerland, the Palestinian Ministry of Health, Helsinki Committee, and An-Najah National University in Palestine.Findings
The health research system is not well structured, whereas public health research is promising but probably without regulated national policies. Most experts emphasised that governance is not clearly framed in managing research functions, whereas public health research activities are most likely scattered and individually driven. There is a consensus that the concept of the health research system is misunderstood and that the system is underperforming because of various problems such as resource insufficiency. Research is also not fundamentally at the heart of the political agenda or itemised in central budgets. Besides workforce scarcity with poor incentives and infrastructure, priorities in public health research are inconsistent and efforts are uncoordinated with poor multidisciplinary research. Dissemination and application of the public health research agenda among stakeholders are lacking. The research culture seems to be insufficiently cultivated. The international support to the public health research system is inconspicuous although some initiatives have been successful. The overall environment in the occupied Palestinian territory formed one of obstacles of the public health research system. Precious opportunities are proposed to strengthen public health research system synergistically through best strategies.Interpretation
The occupied Palestinian territory is a fertile place for growth of public health research system activity. Development actions should therefore be taken to get the system materialised by reactivating a unified governance body that cooperatively manages the national policies, capacities, priorities, research utilisation, and application of the public health research system.Funding
The Swiss Federation and Swiss Tropical and Public Health Institute. 相似文献Methods: Philippine Academy of Rehabilitation Medicine physiatrists conducted a project of 23 emergency basic relief and medical aid missions in response to Super Typhoon Haiyan from November 2013 to February 2014. The final mission was a medical aid mission to the inland rural community of Burauen, Leyte. Summary data were collected, collated, and tabulated; project and mission evaluation was performed.
Results: During the humanitarian assistance project, 31,254 basic relief kits containing a variety of food and non-food items were distributed and medical services including consultation, treatment, and medicines were provided to 7255 patients. Of the 344 conditions evaluated in the medical aid mission to Burauen, Leyte 85 (59%) were physical and rehabilitation medicine conditions comprised of musculoskeletal (62 [73%]), neurological (17 [20%]), and dermatological (6 [7%]) diagnoses. Post-mission and project analysis resulted in recommendations and programmatic changes to strengthen response in future disasters. Physiatrists functioned as medical providers, mission team leaders, community advocates, and in other roles.
Conclusions: This physiatrist-led humanitarian assistance project met critical basic relief and medical aid needs of persons impacted by Super Typhoon Haiyan, demonstrating significant roles performed by physiatrists in response to a large-scale natural disaster. Resulting disaster programing changes and recommendations may inform a more effective response by PARM mission teams in the Philippines as well as by other South-Eastern Asia teams comprising rehabilitation professionals to large-scale, regional natural disasters.

Project analysis indicates that medical mission teams responding in similar settings may expect to evaluate a significant number of physical medicine and rehabilitation conditions.
Medical rehabilitation with participation by rehabilitation professionals including rehabilitation doctors is essential to the emergency medical response in large-scale natural disasters.
